Hopeful Realism

When a seasoned professional at the height of his career can be humble, realistic, and yet hopeful, it makes room for the next generation to expand into the profession. Rosenfeld is 'in' the medical model, but not 'of' the medical model. Briefly stated, he can look at the necessity yet limitations of evidence-based therapy, and transcend those limitations when needed to allow therapy to be the art form it must be to be truly effective.

This book is packed with practical wisdom that can transfer easily to the treatment room, and can be applied to working with adults as well as children and adolescents. A lot to digest? Yes. But for the newly practicing intern, this book has the information needed to answer the barrage of complex questions they are facing daily. Licensed therapists can take inventory and make important upgrades to keep growing. This realistic, inspirational guide is worth several readings.

That was my experience reading this book as an MFT for 2 years now. I only wish I had this book when I was an intern. It would have saved me a lot of stress, as it makes up of for the limitations of supervisors who may be too opinionated at times to be helpful. The author provided me with such a wide spectrum of information with no rigid theory to adhere to--just a relentless pursuit of honest self-examination and critical thinking, with enough real-life information to consider the real questions of therapy. I can benefit from some of the answers the author has come to, but in the end, I have to think things through, come to my own conclusions, and make my own clinical decisions. This book, unlike many others I have read, has helped me to accelerate that process. I estimate that it will take another couple of years to integrate all the information in this book, and in the process, I will become a more complex thinker.

Beyond Evidence-Based Psychotx a real wake up call

Dr. Rosenfeld's book "Beyond Evidence-Based Psychotx: Fostering the 8 Sources of Change in Child & Adolescent Tx" was a real wake up call for me. Lately it seems that governments, insurance companies, and the media have all been hyping "Evidence Based Practices (EBP)." But, when you look into the evidence they use to proclaim they are an EBP, it is full of holes. They don't compare their therapies to traditional forms of therapy. They compare their therapy to the progress of people put on a waiting list, or listening to muzak. Also, anyone who is dual diagnosed or drops out is taken out of their study. This is not the kind of research that put mankind on the moon; it is the kind that sells bottled water. I like the approach of looking for the common factors, regardless of theory, that in the client's opinion, promoted their positive changes.

I used to feel awkward when an intern or graduate student would ask about my theory base. I have amalgamated several strong theories into a toolbox that helps kids become more resilient. Clinicians who are theory purists scare me; it feels like it is their religion. Physicists do not feel the same way about String Theory that purist Cognitive Behavioral Therapists feel about their theory.

I will no longer say I am eclectic. After reading Dr. R's book I will explain that I focus on the Common Factors of different theories that have been researched, and are shown to work.

The second part of the book is a 'day in the life' of a child psychologist. Dr. Rosenfeld captures the challenges and joys of day to day practice.

Masterpiece of Therapy Guidance

Dr. Rosenfeld has written a masterpiece of therapy guidance for child & adolescent therapists and other mental health professionals of all levels of experience. In a time when research and insurance providers seem to agree on the need for a short-term, cognitive-behavioral, medical model approach to therapy, Dr. Rosenfeld begs to differ. His vast clinical experience along with excellent literature review skills allows him to uncover and explain pertinent issues which free us from these stifling limitations. This book has two parts. Part One introduces and explores factors that produce positive change in therapy, and those which do not. Part Two gives us a chance to spend a day with Dr. Rosenfeld and his clients and see how the forces of change in Part One can be incorporated in treatment. The reader experiences what it is like to be a therapist - behaviorally, intellectually and emotionally.

Part One - Dr. Rosenfeld has identified sources of positive change in psychotherapy. He has done this after filtering through and debunking research on evidence-based treatments that have methodological problems, biases, and political & economic forces which have distorted the findings. This is an accomplishment in and of itself. He reveals interconnected spheres of relationship between the therapist and the client which when understood and used well can significantly bolster therapy gains and outcomes. He eloquently delineates the art of therapy and renders it understandable to the novice, as well as giving the seasoned therapist many "ah-ha" moments to ponder and plan around. He explores the therapist's contributions to treatment: using the therapeutic relationship as a treatment tool, how to keep clients in treatment so they can benefit, managing treatment, and engaging clients by helping them seek greater happiness. He does this all with amazing compassion, skill, and clarity. His extreme empathy for the client, backed by an extensive, readable literature review, gives the reader a satisfaction that this information is endlessly useful and very timely. He ends Part One with a riveting jewel of a chapter that reflects on therapy as a career choice, and whether it was a good one. Through this thorough illustration and explanation of therapy's complex dynamics, Dr. Rosenfeld is easy to read, easy to relate to, and even humorous at times.

Part Two - In this section the author presents case studies using a typical day of a therapist, the pace of a therapy session, and 3 month follow-ups. This puts more of an experiential perspective to the concepts presented in Part One. Dr. Rosenfeld, however, does use a lot of clinical examples throughout his book that are helpful and interesting.

Dr. Rosenfeld gives us a profound and well-grounded approach to successful treatment outcomes. His personal clinical experience coupled with good research guidelines makes this a must read for anyone working with the children and adolescent population, or adults for that matter, in the mental health field. Having been an inpatient social worker for 5 years and an outpatient therapist with youth and their caregivers for 11 years, I appreciate this well presented insight. Thank you Dr. Rosenfeld - brilliant work!
